Job summary

Signify Health, part of CVS Health, is seeking a per diem physician to perform in-home health evaluations. You will visit members at their homes, review histories, and perform physicals with a focus on personalized care. The role supports virtual visits and local travel within licensed states.

The position is part-time (12 hours per week) with an hourly base pay of $105.00 and additional incentives. Applicants must hold an active medical license and preferably multi-state privileges.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred to support travel across states.

Responsibilities

• Conduct in-home health evaluations for plan members.
• Engage with individuals and families to address health needs.
• Review medication and medical history, perform physical evaluation.
• Coordinate with health plan for referrals and next steps.
• Travel locally to homes and perform virtual visits as needed.
• Provide diagnostic and preventive services per protocol.
