Job summary

Signify Health, part of CVS Health®, seeks a Per Diem Physician to conduct in-home health evaluations for plan members. You’ll visit patients in their homes, perform medical history reviews, and carry out physical evaluations using an iPad workflow. No prescribing or lab orders are required unless directed by the plan.

The role is part-time with estimated weekly hours of about 12, offering a base hourly rate with performance incentives; travel within licensed states is required as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multistate licenses preferred.

Responsibilities

• Visit patients in-home for health evaluations and history reviews.
• Perform physical evaluations and coordinate with health plans for tests when ordered.
• Engage with patients and families to address health questions and care needs.
• Travel locally and within licensed states for visits.
