Qualifications

• Active, unrestricted medical license(s) in the coverage area(s) and board certification where applicable.
• MD or DO degree and license to practice in the United States.
• Ability to travel locally within licensed states for in-home evaluations.

Responsibilities

• Provide in-home health evaluations to members in their own residences.
• Conduct medication and medical history reviews, physical evaluation and any ordered diagnostics.
• Coordinate with health plans and care teams to bridge gaps in care.
• Travel locally to visits and perform virtual visits as needed.
