Job summary

Matrix Medical Network is recruiting nurse practitioners to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. Visits last about an hour and focus on medical history, social context, and risk factors to close care gaps.

We offer flexible scheduling, with visits Sunday–Saturday and pay tied to completed assessments. Benefits include mileage reimbursement, 401(k), referral bonuses, ongoing education, and licensure support.

Qualifications

• Must hold NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family or related NP specialty.
• BLS/ACLS certification is required.
• Minimum 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and automobile insurance.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family caregivers.
• Coordinate patient education and follow-up with PCP.
• Provide services at member residences (home or SNF).
