Job summary

Signify Health is seeking a Part Time Physician to perform In-Home Health Evaluations, visiting patients in their own homes and helping bridge gaps in care. Each 45‑minute visit includes review of meds, history, a physical exam, and optional tests; you are not required to prescribe or alter treatment.

You will use an iPad with a simple workflow, travel locally within licensed states, and also conduct virtual visits when needed.

Qualifications

• Active, unrestricted license in PA (or eligible in other coverage areas).
• Multi‑state licenses preferred.

Responsibilities

• Engage with patients during 45‑minute in‑home visits, reviewing medications and medical history and performing a physical evaluation.
• Provide Diagnostic & Preventive Services as indicated, without prescribing new medications or altering the patient’s current regimen.
• Travel locally within licensed states; conduct virtual visits as needed using Signify Health workflows.

Education

MD or DO license in Pennsylvania
