Job summary

Signify Health, in partnership with CVS Health, seeks a full-time physician to perform in-home health evaluations for plan members in their own homes. You will perform medical history reviews, physical exams, and determine preventive care needs while using an iPad and clinical workflow.

The role emphasizes compassionate, personalized care, with travel across licensed states and virtual visit capabilities as needed. Comprehensive benefits apply to full-time clinicians.

Responsibilities

• Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Be flexible to travel locally and within licensed states, as needed
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
