Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations to members in their own homes. The role emphasizes compassionate patient engagement, clinical assessment, and coordination with health plans for appropriate care access.

You will travel locally, conduct annual evaluations, and participate in a comprehensive benefits package. Licensure in relevant states is required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Visit members in their homes to perform health evaluations and answer health questions.
• Conduct medication and medical history review and physical evaluation.
• Coordinate with health plans to ensure appropriate care referrals when needed.
• Conduct virtual visits when required and travel locally as needed.
