Job summary

Signify Health, part of CVS Health, is seeking a full-time physician to perform in-home health evaluations at patients’ homes. The role includes a comprehensive review of medical history, physical examinations, and diagnostic testing as ordered by the health plan.

You will join a national network of clinicians, visit patients in their homes, and support bridging gaps in care while traveling locally and within licensed states as needed. Prior in-home clinical experience is valued.

Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Multi-state licenses (preferred).
• Physician with in-home evaluation experience is a plus.

Responsibilities

• Conduct in-home health evaluations, including medical history review and physicals.
• Engage with patients one-on-one to address health questions and plan care.
• Perform diagnostic tests as indicated by health plan and coordinate referrals.
