Signify Health, part of CVS Health, is seeking a Part Time Physician to provide in-home health evaluations in members' homes. The role involves patient engagement, health history review, physical evaluation, and coordinating with health plans.

The clinician uses an iPad-based workflow and is not required to prescribe medications or order labs. The position supports flexible visiting schedules with local travel and virtual visits as needed.

Qualifications

• MD or DO with active, unrestricted license in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for plan members.
• Engage with patients in their homes for ~45 minutes and answer health questions.
• Use iPad and clinical workflow; no prescribing or ordering tests unless plan requires.
