Job summary

Matrix Medical Network is seeking a licensed Nurse Practitioner to perform comprehensive in-home health assessments for adult and geriatric patients. The role includes reviewing medical history, diagnoses, treatment plans, and educating patients and families in their home or SNF settings.

The ideal candidate holds state NP licensure, board certification in an adult-gerontology or related specialty, and current BLS/ACLS. Flexible scheduling and opportunities for career growth are offered.

Qualifications

Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
Current NP licensure in state of practice to include prescription authority or ability to obtain as needed.
AANP, AACN, or ANCC board certification in appropriate NP specialty.
Current BLS, ACLS or CPR certification.
1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation.
Administer and order point of care and lab-based testing as needed.
Work with physicians, case managers, social workers, family members, caregivers, and other medical staff as appropriate.
Collaborate with PCP on patient education and follow up.
Provide services at the member’s residence, including homes or Skilled Nursing Facilities.
