Job summary

Matrix Medical Network seeks skilled nurse practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to improve outcomes. You will order tests, collaborate with physicians and care teams, and provide patient education at the member's residence, including homes or skilled nursing facilities.

Strong licensure, board certification, and a commitment to compassionate, evidence-based care are essential.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult/ Gerontology/ Family or Emergency NP specialties.
• Current BLS/ACLS certification.
• At least 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance.
• Proficient with health records software.

Responsibilities

• Conduct adult/geriatric comprehensive health assessments and document per Matrix guidelines.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family/caregivers.
• Coordinate with PCP for education and follow-up.
• Provide services at members’ residences, including homes or SNFs.

Education

Master’s Degree or commensurate experience

NP licensure in state of practice

Board certification (AANP/AACN/ANCC)

BLS/ACLS certification

1 year NP experience preferred
