Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform comprehensive in-home health assessments, reviewing health history, medications, and social factors to identify gaps in care. Visits can occur in homes or facilities and are scheduled to fit your availability.

The role emphasizes collaboration with PCPs and care teams, with training, preceptorship, continuing education, and potential state licensure support provided by the company.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in relevant NP specialty (AANP/AACN/ANCC).
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred, new grads welcome.
• Valid driver's license for residence state.
• Automobile insurance coverage as required for residence state.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, and referrals per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers during home visits.
• Coordinate with PCP on patient education and follow-up.
• Provide services at members' residence, including homes or skilled nursing facilities.
