Zusammenfassung

Matrix Medical Network seeks qualified Nurse Practitioners to perform comprehensive in-home health assessments across the care continuum. Each visit lasts up to one hour and focuses on medical history, medication adherence, and overall health risks in the member’s residence.

Providers collaborate with physicians, case managers, and family members to identify care gaps, perform testing as needed, and deliver actionable education.

Qualifikationen

• Must have NP licensure in state with prescriptive authority or ability to obtain
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply

Aufgaben

• Conduct adult/geriatric health assessments, including medical history, diagnosis, treatment, health education, referrals, and documentation
• Administer and order point-of-care and lab testing as needed
• Collaborate with physicians, case managers, social workers, and family members
• Coordinate patient education and follow-up with PCP
• Provide services at members’ homes or in Skilled Nursing Facilities
