Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and risk factors at patients' residences. You will collaborate with PCPs, case managers, and care teams to close care gaps and improve outcomes.

Eligible candidates should have NP licensure in their state, prescriptive authority, and board certification (Adult, Gerontology, Family, Emergency or Acute Care).

Qualifications

Master’s degree and NP licensure in state of practice.
Current NP licensure with prescriptive authority.
Board certification in Adult, Gerontology, Family, or Emergency.
Current BLS, ACLS, or CPR certification.
1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

Perform adult/geriatric health assessments including medical history, diagnoses, and follow-up.
Order and administer point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, and families.
Coordinate patient education and follow-up with PCPs.
Provide services at patients' residences (home or SNFs).
