Qualifikationen

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice.
• Board certification in Adult, Family, or Gerontology NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged.
• Valid state driver's license and car insurance coverage.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication skills.
• Ability to travel to members' homes and facilities.

Aufgaben

• Conduct adult/geriatric assessments including medical history and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, and staff as appropriate.
• Provide in-home services at members' residences or SNFs.
• Document findings clearly following Matrix guidelines.
