Zusammenfassung

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical history, medications, adherence, and social factors to identify care gaps and coordinate with physicians and care teams.

Responsibilities include conducting geriatric assessments, ordering tests, and providing patient education. You’ll operate in homes or SNFs with flexible scheduling and competitive pay, supporting older adults and at-risk individuals across the nation.

Qualifikationen

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure with prescriptive authority
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred, new grads encouraged to apply
• Valid state driver’s license for the state of residence
• Automobile insurance coverage requirements for the state of residence
• Strong computer skills with health record software
• Excellent verbal and written communication skills

Aufgaben

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members, and other care staff
• Coordinate patient education and follow-up with PCPs
• Provide services at the member’s home or Skilled Nursing Facilities
