Matrix Medical Network seeks a Nurse Practitioner to conduct comprehensive in-home health assessments. You will review medical history, medications, and social determinants, coordinating with physicians and care teams to close care gaps.

Flexibility with scheduling and travel to members' homes is essential. The role requires NP licensure in the practice state, board certification, and strong communication skills.

Qualifikationen

• Must hold NP licensure in the state of practice with prescriptive authority where required.
• Demonstrated professional nursing licensure and board certification in an applicable NP specialty.
• Strong clinical skills with documentation and care coordination experience.
• Willingness to obtain licensure in neighboring states as needed.
• Proficient with electronic health records and health information systems.

Aufgaben

• Perform adult/geriatric assessments including medical history and diagnoses per Matrix guidelines.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate patient education and follow-up with PCPs.
• Provide in-home assessments at member residences or SNFs.

Kenntnisse

Excellent verbal and written comms

Strong computer skills

Driver's license

Ausbildung

Master’s Degree or commensurate NP licensure

Current NP licensure with prescriptive authority

Board certifications: AANP/AACN/ANCC

BLS/ACLS/CPR certification
