Lifespan Therapy is seeking a licensed Physical Therapist to provide in-home care for geriatric patients across East County San Diego. You will evaluate patients, create individualized plans, and implement functional interventions to enhance mobility and independence.

Qualifications

• Requires valid California Physical Therapist license.
• CPR or BLS certification may be required.
• Driver's license and auto insurance.
• Physical + TB test within the last year.
• Professional liability and business license if applicable.

Responsibilities

• Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.
• Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.
• Collaborate for Optimal Outcomes: Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.
• Document Efficiently: Use one simple EMR system for notes (with tools available to minimize documentation time!)
