Job summary

Lifespan Therapy is seeking a Home Health Physical Therapist to provide in-home geriatric care across Southern California. You will conduct comprehensive evaluations, develop individualized plans of care, and deliver functional strengthening, gait and balance training, all with autonomy and flexibility.

You set your own hours and manage your caseload, while our app handles referrals, scheduling, and invoicing to support your clinical work.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
