Job summary

VNA Health Group's Visiting Physician Services (VPS) seeks a Physician Assistant to deliver high-quality, home-based primary care to older adults in New Jersey. The role involves patient assessments, care planning, and coordination with a multidisciplinary team during home visits and telehealth.

Ideal candidates hold NJ PA licensure with prescriptive authority, national PA certification, and 2+ years in adult or geriatric care, plus strong communication and computer skills.

Qualifications

• Completion of Physician Assistant (PA) accredited program.
• Licensure as a Physician Assistant (PA) in NJ with prescriptive authority; maintain a CDS and DEA.
• National certification as a PA with AAPA.
• Experience providing care to Adult or Geriatric patients.
• 2+ years of Physician Assistant experience and/or significant experience as a registered nurse.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Assesses, diagnoses and develops appropriate plan of care to treat homebound patients.
• Monitors the patient's condition and responsiveness to the treatment provided.
• Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations.
• Participates in organizational wide QAPI program and attends agency sponsored in-service programs.
• Adheres to agency infection control and safety policies.
