Job summary

VNA Health Group’s Visiting Physician Services (VPS) seeks a Nurse Practitioner to deliver high‑quality, home‑based primary care to older adults who are homebound or medically complex. As part of the Parker Advanced Care Institute, the NP will assess and manage treatment plans with an interdisciplinary team.

The role requires NJ APN licensure and geriatric certification, with strong communication and computer skills. Travel within New Jersey is required.

Qualifications

• Master’s degree in Nursing from an NLN-accredited program.
• APN license in NJ with prescriptive authority; CDS and DEA.
• National certification as Adult or Gerontologic Nurse Practitioner.
• 2+ years APN or RN experience.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Assess, diagnose and develop plan of care for homebound patients.
• Monitor patient condition and response to treatment.
• Act as agency representative with patients, families and other health agencies.
• Participate in organizational-wide QAPI program and attend agency sponsored in-service programs.
• Adhere to infection control and safety policies.
• Adhere to absenteeism and appearance policies.
