Job summary

VNA Health Group, through Visiting Physician Services, is seeking a physician to deliver high‑quality in‑home primary care to homebound patients in New Jersey, focusing on comfort, safety and whole-person care.

You will work with an interdisciplinary team, document in the EMR, comply with health regulations, and travel to patients' homes to provide compassionate, patient‑centered care that improves quality of life.

Qualifications

• Licensed as a physician in New Jersey with education and experience in primary care settings.
• Geriatric experience recommended.
• Board certified in family practice or internal medicine.
• Active CDS and DEA license.
• Excellent verbal and written communication skills.
• Excellent organizational and interpersonal skills.
• Car and valid NJ driver’s license.

Responsibilities

• Provide direct medical care and consultation to homebound primary care patients within established productivity guidelines.
• Monitor patient conditions and adjust treatment plans based on clinical needs and responsiveness.
• Serve as a compassionate, professional representative of the agency to patients, families, and community partners.
• Collaborate with colleagues across the interdisciplinary team.
• Contribute to both formal and informal staff education.
• Maintain accurate, timely documentation in the Electronic Medical Record.
• Ensure compliance with NJ Department of Health regulations and all relevant licensing and accrediting requirements.
