Qualifications

• Master’s degree in Nursing from an NLN-accredited program.
• NJ Advanced Practice Nurse license with prescriptive authority; CDS and DEA.
• National certification as an Adult or Geriatric Nurse Practitioner.
• 2+ years APN or RN experience.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Supports organization’s mission by striving for excellence with positive interpersonal relationships.
• Assesses, diagnoses and develops plan of care for homebound patients.
• Monitors patient condition and response to treatment.
• Acts as agency representative with patients, families and other health agencies.
• Participates in organizational wide QAPI program and in-service trainings.

• Adheres to absenteeism and appearance policies.
