Job summary

LiveWell Homecare Agency, LLC is seeking a qualified NP or PA to perform comprehensive in-home health assessments for adult and geriatric members, up to one hour per visit. Responsibilities include reviewing medical history, medication adherence, social factors, conducting tests, and documenting findings.

The role involves coordinating with physicians, case managers, and caregivers, traveling to members' homes and Skilled Nursing Facilities, and providing patient education and follow-up.

Qualifications

• Master’s degree or equivalent with NP/PA licensure.
• Prescriptive authority licensure and relevant certifications.
• CPR/BLS/ACLS certifications.
• Minimum of one year NP or PA experience.
• Willingness to travel extensively, including overnight stays.
• Valid driver’s license and insurance; strong computer and communication skills.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history and health education.
• Order and administer point-of-care and lab tests.
• Coordinate with physicians, case managers, social workers, and caregivers.
• Follow up with primary care physicians and provide patient education.
• Work primarily in members’ residences, including homes and SNFs.
