Matrix Medical Network seeks a licensed NP to conduct comprehensive in-home health assessments. Visits last about an hour and focus on medical history, medications, and social determinants of health. You will document findings per Matrix protocols and coordinate with physicians and care teams.

Responsibilities include ordering tests, collaborating with PCPs, and providing patient education and follow-up, enabling high‑quality care at patients’ homes or facilities.

Qualificações

• Must hold a current Nurse Practitioner license and be eligible to practice in the state.
• Master’s degree in nursing or commensurate experience with NP certification.
• Board certification in Adult, Family or gerontology specialties preferred.
• Current BLS/ACLS or CPR certification required; valid state driver’s license.

Responsabilidades

• Perform adult and geriatric health assessments during home visits.
• Review medical history, medications, and social factors; document findings per Matrix guidelines.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with PCPs, case managers, and caregivers; provide education and follow-up.
