Matrix Medical Network provides in-home comprehensive health assessments to improve care for older adults and at-risk individuals. Providers conduct visits lasting up to one hour, reviewing health history, medications, and risk factors at the member’s home or SNF.

Responsibilities include nursing and medical assessments, ordering tests as needed, collaborating with PCPs and care teams, and delivering clear documentation following Matrix guidelines.

Qualifications

• Master’s degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult/ Gerontology/ Family/ Emergency NP specialties; CPR/BLS/ACLS.

Responsabilités

• Conduct adult/geriatric assessments including medical history, diagnoses and treatment, health education, referrals and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide services at the member’s residence (home or SNF) and coordinate follow-up with PCP.
