Job summary

Centerstone seeks an Outreach/Home Visitor to deliver in-home services to enrolled families in Cannon County and surrounding areas. The role focuses on building trust, improving family functioning, and linking families to support services while adhering to HFA/ GGK frameworks.

You will conduct weekly home visits, use standardized tools, and help families create and follow a Family Service Plan. A bachelor’s degree and related experience are required.

Qualifications

• Bachelor’s degree in psychology, social work or related field required.
• Two years of professional experience working directly with youth and families preferred.
• No certification required.

Responsibilities

• Initiate and maintain regular, long‑term contact with families.
• Deliver family-centered, strength‑based home visiting services.
• Support goal setting and implement Family Service Plan activities.
• Administer FROG Scale and screening tools as needed.
• Prepare monthly, quarterly and annual reports.
• Maintain accurate family records and complete required documentation.
• Attend mandatory trainings and meetings.
• Provide case management and weekly GGK/HFA activities.
