Responsibilities

Provide evidence-based home visiting to families and maintain a caseload per program standards.
Document services and track progress in required systems and forms.
Ensure well-child visits and immunizations are up to date per schedules.
Screen for mental health needs and refer for services when appropriate.
Maintain family-centered case management and connections to community resources.
