Job summary

Northeast Family Services seeks a Case Manager to serve as a liaison for treatment teams, clients, families and collateral providers. You will maintain caseloads, coordinate with DCYF on referrals, and develop measurable treatment plans with families.

The role requires a Bachelor’s in a related field, at least two years of relevant experience, and a valid driver’s license. Evening visits and in-home services are common, with opportunities for supervision for licensure.

Qualifications

• Bachelor's degree in a related field.
• Two years minimum experience working with children, youth and families in home or community settings.
• Valid driver's license with reliable access to a vehicle.
• Authorized to work in the U.S. (no visa sponsorship).
• Access to high speed, secure internet required.

Responsibilities

• Maintains assigned caseload and ensures proper implementation of in-home services per DCYF referrals.
• Collaborates with DCYF to respond to referrals, create treatment plans, participate in meetings, and document accordingly.
• Develops and aligns treatment plans with measurable goals in partnership with families and DCYF.
• Complies with documentation requirements including assessment and reporting for the court system as needed.
• Provides parent education and supervised visitation to children, adults and families in home and community settings.
• Offers parent coaching and support, including healthy parenting strategies and addressing healthcare/education needs.
• Interfaces with DCYF, NH court system, schools and other treatment team members to ensure quality service delivery.
• Assesses client risk and consults with clinical supervisor on safety concerns.
• Transports clients, including car seat/booster installation as needed.
• On-call rotations for family support; evening hours are required.
• Documentation of all clinical contacts and visits via Electronic Health System.
• Other duties as assigned.

Education

Bachelor's Degree in social work, counseling, psychology or related human services field
