Job summary

Northeast Family Services is a premier mental health provider serving children, youth, and families across New Hampshire. We are seeking a Case Manager to act as a liaison for the treatment team, clients, families, and collateral providers.

The role involves in-home services, treatment planning, documentation, and court reporting in line with DCYF referrals. The position requires a Bachelor’s degree in a related field, a valid driver’s license, and authorization to work in the U.S.

Qualifications

• Bachelor’s Degree in social work, counseling, psychology or related human services field.
• Two years minimum experience with children, youth and families in home or community setting preferred.
• Valid driver’s license with reliable and consistent access to a vehicle.
• Applicants must be authorized to work in the U.S. We are unable to sponsor or take over sponsorship of an employment visa at this time.
• Access to high speed, secure internet required.

Responsibilities

• Maintains assigned caseload and ensures proper in-home service delivery per referrals by DCYF.
• Collaborates with DCYF to respond to referrals, create treatment plans, participate in treatment meetings, and provide documentation.
• Develops and collaborates on treatment plans with measurable goals with families and DCYF.
• Adheres to documentation requirements including assessments and reports.
• Participates in court reporting as requested.
• Provides parent education and supervised visitation in home/community-based settings.
• Offers parenting strategies and support including meeting healthcare and educational needs of children.
