Job summary

Northeast Family Services in New Hampshire is seeking a Case Manager to join our clinical team. The role involves serving as a liaison for the treatment team, clients, families and collateral providers, with emphasis on in-home services, treatment planning and documentation.

Responsibilities include maintaining caseload, coordinating with DCYF, conducting assessments and reports, and offering parent education and supervised visitation.

Qualifications

• Bachelor’s Degree with a major in social work, counseling, psychology or related human services field
• Two years minimum experience working with children, youth and families in a home or community setting preferred
• Valid driver’s license with reliable and consistent access to a vehicle
• Applicants must be authorized to work in the U.S. We are unable to sponsor or take over sponsorship of an employment visa at this time.
• Access to high speed, secure internet required.

Responsibilities

• Maintains assigned caseload and ensures proper implementation of in-home services.
• Collaborates with DCYF to respond to referrals, create treatment plans, participate in treatment meetings, and provide documentation.
• Develops and collaborates on treatment plan formation with measurable goals with families and DCYF.
• Documents all clinical contacts and documentation through the Electronic Health System.
• Provides parent education and supervised visitation to children, adults, and families in home and community-based settings.
