Job summary

Elwyn seeks a Case Manager to maintain regular contact and visits with families on their assigned caseload to work on the family’s goals based on a plan created in collaboration with the family and the DHS worker, when applicable, based on the reason for the referral/complaint. The Case Manager works with families to provide support and counseling in the areas of appropriate parent-child interaction.

Family Empowerment Services case managers provide bi-monthly home visits to promote family

Qualifications

• Bachelor’s degree in social work or related human services field required.
• Two years’ experience in social work or related case management/family welfare.
• Experience with children birth to 17 and urban families preferred.
• Knowledge of community resources and non-profit human services helpful.
• Must have valid driver’s license and reliable vehicle in the United States.

Responsibilities

• Create a plan for each family on caseload after DHS referral.
• Collaborate with families to set goals and access services (medical, housing, etc.).
• Maintain regular home visits to check-in and provide support.
• Assess family risk using FAST and document findings.
• Maintain professional, family-centered relationships and confidentiality.
• Document interventions and progress in agency records on schedule.
