PACE Southeast Michigan is seeking a Direct Home Care Worker to support participants in Westland, MI. You will provide in-home and community-based care, assist with daily activities, transportation, and day health center visits, following each participant's Care Plan.

The role emphasizes helping participants achieve independence, maintaining safety, and documenting visits. Requires a high school diploma or GED, at least two years of relevant experience, and completion of recognized Direct Care

Competenze

• Minimum High School Diploma or GED is required.
• Must have at least 2 years of relevant work experience.
• Must complete formal training program (Direct Care, CNA, MA) outside PACE SEMI if needed.

Mansioni

• Provide in-home or community-based supportive services per participant care plan.
• Assist with activities of daily living, meal prep, and transportation support.
• Document care provided accurately and timely on each visit.
• Report changes or complaints to supervisor promptly.
• Maintain infection control and safety procedures.
