Job summary

PACE is seeking a Direct Home Care Worker to provide in-home and community-based support in Detroit, MI. This part-time role supports frail elders and individuals with dementia within their Care Plan, including personal care, meal assistance, and accompanying to appointments.

The position emphasizes documentation accuracy, timely communication with RNs and nurses, and adherence to scope of practice. Training and immunization requirements apply.

Qualifications

• Must have minimum of 2 years of relevant work experience.
• Must have completed a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.).
• Must be medically cleared for communicable diseases and up-to-date on immunizations.

Responsibilities

• Provide direct home care in the participant's home or community setting.
• Assist with activities of daily living, meal preparation, and grooming.
• Assist with toileting and continence management and simple treatments as needed.
• Provide transportation and accompany to appointments or day health center.
• Maintain accurate, timely documentation of care and progress; report deviations to supervisor.
