Job summary

PACE Southeast Michgan seeks a Direct Home Care Worker for part-time, in-home and community-based support in Eastpointe, MI. The role follows a participant-specific care plan and requires documenting each visit, coordinating with RNCM, and reporting deviations from schedule.

Responsibilities include personal care, meal assistance, mobility and transportation support, and maintaining a respectful, safe environment for frail elders, including those with dementia.

Qualifications

• Minimum High School Diploma or G.E.D. required.
• Must have minimum of (2) years of relevant work experience.
• Must have completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.

Responsibilities

• Provide in-home or community-based care per the participant's care plan.
• Assist participants with activities of daily living and basic needs.
• Support with meal preparation, toileting, shopping, and transportation as needed.
• Maintain accurate timely documentation of all visits and encounters per policy.
• Respond to emergencies and communicate progress to the RNCM or nurse.
