Job summary

PACE Southeast Michigan in Clinton Township seeks a Direct Home Care Worker to provide in-home and community support per the participant's care plan. The role includes personal care, meal assistance, transportation, and home health tasks across home, community settings, and day health centers.

Required qualifications include a high school diploma or GED, at least two years of relevant experience, and completion of a formal training program outside PACE SEMI.

Qualifications

• Minimum High School Diploma or GED required.
• Must have a minimum of 2 years of relevant work experience.
• Must have completed a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.
• Participates in annual required in-service training including infection control, TB testing, safety training and BLS.

Responsibilities

• Provide direct in-home care per the participant's care plan.
• Assist with personal care, activities of daily living, meals, toileting and transportation.
• Support participants in home, community settings, day health centers and during appointments.
• Accurately document visits and communicate progress to the RNCM or clinic nurse.
• Escalate and report all deviations from schedule or issues to supervisor.
