Job summary

The Direct Home Care Worker at PACE Southeast Michigan provides in-home and community-based support to participants, assisting with daily activities, meals, transportation, medication reminders, and homemaking tasks.

Under the participant's Care Plan, the worker helps expand independence, supports families, documents visits accurately, and reports issues promptly; driving within the catchment area and maintaining required immunizations are expected.

Qualifications

• Minimum High School Diploma or GED required.
• Minimum of 2 years relevant work experience.
• Completed formal training in Direct Care, Nurse Assistant, or Medical Assistant.

Responsibilities

• Provide in-home and community-based support and personal care.
• Assist with activities of daily living, meals, toileting, and transportation.
• Support participant independence within Care Plan and dementia unit as needed.
• Accurate timely documentation of visits and progress; report deviations to supervisor.
