Nashua Center in Derry, NH seeks an Early Intervention Service Coordinator to join the FCESS program serving children birth to three and their families. You will coordinate IFSP services, work closely with a multi-disciplinary team, and support families at home and in community settings.

The role emphasizes strong organizational, communication, and problem-solving skills, with driving required for home visits and community outreach.

Qualifications

• Associate's degree in early childhood or case management; Bachelor's degree preferred.
• Strong organization and time management skills with ability to meet timelines.
• Experience working independently and as part of a team.
• Strong verbal and written communication skills.
• Ability to communicate with diverse groups and adapt to needs.
• Valid driver's license, insurance, and access to a reliable vehicle.
• Willing to undergo a criminal record search.

Responsibilities

• Guide families through the FCESS process from evaluation to discharge.
• Coordinate all services per the IFSP plan for each child.
• Serve as primary contact for families and help access community/state resources.
• Provide in-home support and administrative assistance to therapy staff related to the IFSP.
• Assist families through transitions to local preschool.
• Monitor overall program caseload.
