Job summary

MECA Therapies LLC in Espanola, NM, is seeking a compassionate Family Service Coordinator (FSC) to support families with children birth to three in Early Intervention. You will navigate IFSPs, coordinate services, and connect families with resources in home and community settings to promote developmental goals.

The role emphasizes collaboration with families, therapists, and community partners, requiring strong communication, confidentiality, and organizational skills.

Qualifications

  • Bachelor's degree in one of the listed fields or two years' experience serving individuals with disabilities and/or families.
  • Ability to complete required Family Service Coordination training within one year of hire.
  • Strong organizational and record-keeping skills.
  • Effective verbal and written communication skills.
  • Strong interpersonal and collaboration skills.
  • Ability to maintain confidentiality and comply with HIPAA requirements.
  • Proficiency with Microsoft Windows and electronic documentation systems.
  • Valid driver's license, reliable transportation, and automobile insurance.

Responsibilities

  • Serve as the primary contact and support for families enrolled in Early Intervention services.
  • Explain the Early Intervention process, family rights, safeguards, and resources.
  • Gather information on family concerns, priorities, resources, and outcomes.
  • Coordinate evaluations, assessments, eligibility, and service planning.
  • Assist families in developing and preparing IFSPs.
  • Coordinate and monitor Early Intervention services identified on the IFSP.
  • Connect families with community resources, medical providers, and supports.
  • Facilitate TTCMs, IFSP meetings, reviews, and transition activities.
