CareSource is seeking a Care Delivery RN in Massachusetts to collaborate with an interdisciplinary team, delivering primary and home-based care to high-risk patients with complex medical, behavioral, and social needs. You will coordinate care, perform RN assessments, and ensure timely follow-ups and documentation within policy guidelines.

Travel to patient homes and community sites is required, and you will work with PCPs, providers, and care managers to close care gaps and improve outcomes.

Formación

• Associates degree in nursing required
• Bachelor's degree in nursing preferred
• Five years of experience as Registered Nurse in a high touch clinical environment or home care required
• Two years of experience caring for patients/members with complex medical, behavioral health, and social needs required
• Experience with electronic medical record strongly preferred
• Current Massachusetts RN license required

Responsabilidades

• Delivers care to complex patients, providing care management and care coordination support
• Conducts basic health assessments and provides RN-level care with orders as necessary
• Escalates clinical findings to APC within required timeframes
• Conducts follow-up calls to ensure patient satisfaction
• Assesses quality gap reports and collaborates to close gaps
• Documents visits with clear charting per policies and procedures
• Communicates plan to care team and follow-ups
• Provide care coordination to ensure effective administration of program
• Travel to conduct member, provider and community-based visits as needed
• Support procurement of DME, transportation, LTSS services and community supports
