Qualifications

• 18 years of age or older.
• Ability to obtain CPR/FA certification.
• Ability to clear a DES background check.
• Ability to lift and transfer clients (up to 50 lbs).

Responsabilités

• Coordinate in-home and community-based services.
• Maintain and grow a caseload.
• Conduct intake of new clients.
• Document significant conversations with clients.
