Job summary

Qualifications

• Associate Degree plus two years of Behavioral Health work experience (AHCCCS requirement for BHT).
• 1 year of experience working with housing programs, populations at risk of homelessness, and/or low-income individuals and families.
• Excellent data entry skills (speed and accuracy).
• Proven skills utilizing Microsoft Office Suite.
• Excellent organizational skills.
• Proven strong communication skills - oral and written communications are clear and concise using appropriate style, grammar, and tone.

Responsibilities

• Processes self-referrals and referrals from community agencies by providing initial screenings, intakes, interviews, and eligibility assessment of participants
• Develops individual service plans in partnership with clients; monitors progress and implements follow-up activities
• Provides referrals, assists the client in arranging appointments with community resources or private group counseling sessions and monitors client progress
• Maintains individual service files for clients that include plans and strategies for achievement of housing goals and provides follow up contacts to assess progress
• May drive agency or personal vehicle on company business
