Job summary

CareSource in Massachusetts seeks a Clinical Care Manager to provide monitoring and care management for dually-eligible enrollees with complex needs in home and community settings. You will coordinate with providers, social services, and family members to improve outcomes and quality of life.

The role emphasizes interdisciplinary teamwork, assessments, and proactive care planning with emphasis on continuity and prevention.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required
• Master's degree in social work or mental health counseling and independent license required
• Three (3) years of experience as a Registered Nurse/BH Clinician or One (1) year as RN/BH with two years of complex needs experience as LPN/CHW/MA required
• Prior experience in care coordination or dual-eligible populations preferred
• Medicaid and/or Medicare managed care experience preferred

Responsibilities

• Engage enrollees in home and community settings to build complex care management relationships
• Coordinate between healthcare providers, resources, and enrollees for seamless care transitions
• Perform assessments including Comprehensive Assessment and functional assessments
• Lead the interdisciplinary care team to create holistic care plans
• Oversee utilisation of long-term services and supports to keep enrollees in their chosen location
• Educate members about benefits, services, and chronic care management
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions
• Coordinate with physicians and community partners to avoid duplications of services
• Advocate for enrollees and monitor satisfaction and concerns
• Travel to member, provider and community visits as required
