Job summary

Monogram Health is seeking a Registered Nurse Care Manager to work closely with a multidisciplinary team to manage complex CKD/ESRD patients in home settings. You will build trusting relationships, conduct in-home visits, and coordinate with providers to improve quality of life.

Role requires BLS certification, RN license in the posting state, and 2+ years in care management. Travel to patient homes is required with some training travel.

Qualifications

• 2+ years experience in care management or CKD/ESRD care.
• Graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in the state of the posted location.

Responsibilities

• Develop and adapt care plans with patients and providers.
• Conduct in-home care management visits.
• Monitor biometric data and interventions per protocol.
• Coordinate with pharmacists and prescribers.
