Job summary

Monogram Health is seeking a Care Manager – Registered Nurse to join our in-home care team. You will build trusted relationships with a small panel of patients, develop adaptive care plans with providers, and support patients in achieving a higher quality of life at home.

The role requires 2+ years in care management, RN licensure, BLS certification, and willingness to travel locally for in-home visits; base compensation ranges with strong benefits and potential bonus opportunities.

Qualifications

• Graduate of an accredited School of Nursing and currently licensed as an RN in the posting state.
• 2+ years of care management experience with CKD/ESRD patients.
• BLS certification required; company supports certification on onboarding.
• Willingness to travel locally for in-home visits.

Responsibilities

• Develop and adapt care plans with patients’ medical providers.
• Perform in-home care visits to execute the plan.
• Monitor biometric data and follow protocols for interventions.
• Coordinate with pharmacists and prescribers.
• Educate patients and families about CKD, ESRD, dialysis, and related conditions.
• Support adherence through regular contact and coaching.
