Job summary

Moms In Motion is seeking a Service Facilitator (SF) Case Manager in Virginia. You will visit clients at their homes, guide families through Virginia’s Medicaid Waiver programs, and develop care plans to help people live safer, healthier, and more independent lives.

Ideal candidates have at least 2 years of related experience, no degree required, and a valid driver's license with reliable transportation. Strong communication skills and background checks are required.

Qualifications

• Must live in VA
• 2+ years experience supporting individuals with disabilities or the elderly
• No degree required
• Valid driver’s license and reliable vehicle
• Strong written and spoken communication
• Able to pass background check + provide 2 professional references
• Bonus: Person-Centered Thinking/Planning training

Responsibilities

• Visit clients in their homes locally.
• Guide families through Virginia waiver programs.
• Write care plans and assessments.
• Troubleshoot service authorizations, timesheets, and more.
• Build relationships with families based on respect, patience, and trust.
• Work remotely from your laptop/tablet.
