Moms In Motion, a Virginia-based service facilitation provider, is hiring a Service Facilitator (SF) Case Manager. You’ll visit families in their homes across VA, help them navigate Medicaid Waiver programs, and write plans of care to support safe, healthy, independent living.

We value compassion, organization, and community focus. A valid driver's license and ability to travel locally are required; no degree is necessary, and training is provided.

Qualifications

• 2+ years experience supporting individuals with disabilities or elderly
• No degree required; heart and experience matter
• Tech-savvy enough for email, docs, spreadsheets, portal navigation, video calls
• Valid driver’s license and reliable vehicle (no client transport)
• Great written and spoken communication
• Able to pass background check and provide 2 professional references
• Bonus points for Person-Centered Thinking/Planning training

Responsibilities

• Hit the road locally to visit clients in their homes
• Be the go-to guide for families navigating waiver programs
• Write up plans of care and assessments that actually make a difference
• Troubleshoot challenges like service authorizations and timesheets
• Build lasting relationships with families based on respect, patience, and trust
• Work remotely from your laptop/tablet
