Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will visit clients in their homes across Virginia and help families navigate Medicaid Waiver programs. You’ll write care plans, handle authorizations, and build trusted relationships with clients and families.

A driver’s license and reliable vehicle are required, and there is local travel involved. We value compassionate, organized individuals who can work remotely from a laptop/tablet while delivering excellent support to

• Minimum 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; we care about heart and hands-on experience.
• Proficient with email, documents, spreadsheets, portals, and video calls.
