Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will visit clients in their homes across Virginia, guiding families through Medicaid Waiver programs and ensuring safe, healthy, independent living. The role emphasizes care plans, teamwork, and compassionate service.

Role requires 2+ years supporting individuals with disabilities or the elderly. No degree is required, and a valid driver's license with a reliable vehicle is essential for daily local travel.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
