Job summary

Moms In Motion is hiring Service Facilitators (SF) Case Managers in Virginia. You’ll visit families in their homes, guide them through Virginia’s Medicaid Waiver programs, and help ensure safety, health, and independence.

Roles involve local travel, strong communication, and teamwork. No degree required; 2+ years of relevant experience and a valid driver’s license are expected. Flexible, community-focused work with good benefits.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; experience and heart matter more.
• Tech-savvy for email, documents, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle (no client transport).
• Strong written and spoken communication.
• A background check and two professional references are required.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
